PRICING
It is our goal to provide the highest quality of care and to be able to help as many people as possible.
In order to do accomplish this goal, we have decided not to partner with insurance companies who limit the quality of care we are able to provide, and we kindly ask that you give 24 hours notice for rescheduling and canceling appointments to allow us to be able to offer the spot to someone else.
There is a late cancel fee of $25 per person for adjustment visits and $50 per new patient visits if less than 24 hours notice is given. All no shows/no call/no text appointments will automatically be charged the full visit amount.
Telehealth visits will be charged $50 for both functional medicine new patient and follow up no show visits.
*Exceptions are made for our pregnant mothers who go into labor less than 24 hours before their scheduled appointment.
Payment is due at the time of service.
We accept cash, debit, credit, HSA/FSA accounts, and Zelle.
CHIROPRACTIC
INITIAL VISIT $125
This visit includes a comprehensive health history, neuromusculoskeletal exam, and the first adjustment.
CHIROPRACTIC ADJUSTMENT $45
This visit is for a follow-up chiropractic adjustment for an established chiropractic patient
CHIROPRACTIC ADJUSTMENT FOR 2 PEOPLE: $40 EACH
This pricing is for couples/families who schedule 2 people together at the same visit
CHIROPRACTIC FAMILY PLANS FOR 3+ PEOPLE
This pricing is for families who schedule 3+ people together at the same visit
3-4 People per visit: $35 each
5-6 people per visit: $30 each
7+ People per visit: $25 each
Small Animal Initial Visit $75
This visit includes a history, records review from current vet, a neuromusculoskeletal exam, and an adjustment utilizing veterinary orthopedic manipulation
SMALL ANIMAL ADJUSTMENT $55
Adjustment for canines, felines, and other small animals
*Additional pets treated at the same appt are given a discounted rate of $40. This is also applied to the 1st animal if scheduled at the same appt time as their owner.
FUNCTIONAL MEDICINE
INITIAL VISIT $295
This visit includes:

Comprehensive review of all previous medical records completed by Dr. Carlie before the appointment

Initial 45-60 min appointment with the patient to complete an in-depth health history evaluation

determination and ordering of necessary testing
*any necessary lab, urine, stool, breath, hormone, etc testing is charged by the individual lab companies and is not included in this price

1ST FOLLOW UP

SUBSEQUENT FOLLOW UPS $125
This visit includes:

Review and analysis of all lab work

30 minute appointment to discuss the results of the lab testing

Treatment plan for current condition

Follow-up appointment to determine progress with current treatment plan, answer questions, determine if any changes to the treatment plan are necessary, and order any updated lab testing.
Kinesiotaping $10/$15

DOT PHYSICALS $70
Physical for CDL license holders - initial or renewal.
TELEHEALTH VISITS FOR ESTABLISHED CHIROPRACTIC OR FUNCTIONAL MEDICINE PATIENTS $50
This includes one of the following:

15 minute appointment to discuss treatment recommendations for an illness or injury

15 minute appointment to discuss a new lab work order or lab work results

15 minute appointment to discuss current/new supplement plan
*Additional time will be prorated at $10 per 5 minutes if needed.
PEDIATRIC WEIGHT CHECK OR EAR CHECK $15
This appointment is for current patients wanting either a pediatric weight check, including length if requested, or an ear check outside of a normal adjustment appointment.
